Admissions data is captured almost entirely on paper.  
In fact, 85% of respondents said that less than 3% of their applications are filled out online.


In an ideal world, offering online admission paperwork should encourage families to submit information in an electronic format which would provide much more flexibility for record keeping and documentation.

Although some of the communities we interviewed for our study offer an online admissions form that can be downloaded, filled out and mailed back, this option it is almost never used.

One survey respondent said, “I would like to have a system that is online and will not allow a form to be missed; they cannot continue with the remainder of the agreement until the page is initialed or signed.”

Nearly 60% of the time, facilities find that residents have not completed admissions forms in their entirety.

Most residents enter Long-term Care (LTC) directly from a hospital, usually due to an unplanned event, such as a fall or other health issue.  In this situation, families and/or agents are under extreme stress to pull together all the documents needed to complete the paperwork for their loved one to be admitted.

In addition to the actual application, during pre-admission there can be a dozen or more types of documents that have to be completed. Currently most of these documents are paper based, in different formats, and various configurations. EMR vs. EHR vs. ERR: Which Reduces Readmission Risks?

Long-term care executives often ask how Admit+, an Electronic Resident Record (EER) solution differs from an Electronic Medical Record (EMR) or an Electronic Health Record (EHR).  

It’s easy to mistake the terms as they sound similar and then differentiating them from an electronic forms solution for resident admissions adds an entirely new distinction.


The short answer is that, designed correctly, an EER solution will reduce hospital readmissions risks by only admitting residents you can care for.

First, let’s distinguish between EMR, EHR, and E-forms:

EMRs are digital versions of a doctor’s patient chart and include clinical data related to a specific patient however the information cannot be shared with other members of the care team easily on a computer network outside of that particular practice. In that regard an EMR is not much better than a paper chart.EHRs focus on the total health of the patient— and include all clinician’s notes on a patient and “can be created, managed, and consulted by authorized clinicians and staff across more than one healthcare organization.”

In both cases, the digital record contains only medical data, with limited insurance and personal information thrown in. When a patient is referred for admission to a nursing facility, these records are transferred to a nursing community, usually in paper, but sometimes as a fax (still paper) or a pdf which has to be printed out (again, still paper).  

Admissions staff still have to manually enter all the prospects’ information, not just once, but multiple times to populate the numerous forms required for application and eventually, admittance. E-forms are digital forms that collect information in a common repository for sharing between organizations.
